5. Every person who makes application to the Director of Medical and Sanitary Services in the manner set out in Part III of these regulations for admission to a part of the Register and who complies with the conditions of admission prescribed in that part of the regulations shall upon payment to the Director of Medical and Sanitary Services of a fee of ten dollars be admitted to that part of the Register.
6.-(1) A nurse who desires her name to be retained in the Register for any year subsequent to the first year in which it is included in the published Register, shall on or before the thirtieth of September of the previous year forward to the Director of Medical and Sanitary Services a retention fee of two dollars.
(2) A nurse whose name is included in the Register shall notify to the Director of Medical and Sanitary Services every permanent change of address, and for the purposes of these regulations the address last notified shall be deemed to be the registered address of the nurse.
(3) The Director of Medical and Sanitary Services shall send to the registered address of each nurse whose retention fee has not been received before the 31st October a written notice informing her of the consequence of failure to pay the said fee, and if the nurse fails to forward to the Director of Medical and Sanitary Services the said retention fee by the end of November, her name shall be excluded from the Register as from the begin- ning of the succeeding year unless the Board for any special reason otherwise direct.
7. Where the name of a nurse has been excluded from the Register under the last preceding rule, she shall be entitled to apply to have her name re-included in the appropriate part or parts of the Register. An application for re-inclusion shall be accompanied by a written explanation of the appli- cant's failure to apply for retention in the Register, and a fee of $5 shall be payable for such re-inclusion. If the Board are satisfied with the explanation offered by the applicant, they may re-include her name in the appropriate part or parts of the Register.
8. Within three months of the beginning of the Ordinance, and there- after as soon as possible after the first day of January of each year, the names of all persons included in the Register, with the particulars specified in the First Schedule, shall be published in the Gazette.
PART III.
CONDITIONS OF ADMISSION TO REGISTER.
A.
EXISTING NURSES.
9.-(1) Every person who desires to obtain admission to the Register as an existing nurse shall, within one year after the date on which this part of these regulations come into operation, make application to the Director of Medical and Sanitary Services in Form I set out in the Second Schedule to these regulations, and shall furnish the Director of Medical and Sanitary Services with the names and addresses of at least three persons willing and able to give evidence of good character, one of whom shall be a person, not being a relative of the applicant, who has known her personally for not less than three years, and the others shall be persons such as matrons of hospitals or registered medical practitioners.
(2) The Board shall invite the persons so named by the applicant to furnish testimonials as to her character and professional efficiency in Forms II and III, and containing the particulars, set out in the Second Schedule, and any testimonials so furnished shall be sent direct to the Director of Medical and Sanitary Services.
